Course Description

This focused course provides an in‑depth exploration of pressure ulcers—from pathophysiology and risk assessment through advanced prevention strategies and evidence‑based wound care. 

Students will master staging, the Braden Scale, risk‑reduction techniques, SMARTCARE nursing interventions, and the PRIORITY‑X framework for early red‑flag recognition. 

Through case studies, learners will develop the skills to prevent pressure injury, promote wound healing, and optimize patient safety.

Course Outcomes

Upon successful completion, learners will be able to:

  1. Assess patients’ pressure‑ulcer risk using the Braden Scale and other validated tools.
  2. Differentiate pressure‑ulcer stages I–IV and unstageable wounds to guide appropriate interventions.
  3. Formulate prioritized nursing diagnoses and SMARTCARE‑based care plans that address prevention, wound management, and patient comfort.
  4. Implement evidence‑based repositioning schedules, support surfaces, skin care protocols, and nutritional strategies to prevent and treat pressure ulcers.
  5. Recognize NCLEX‑critical red flags—persistent non‑blanchable erythema, purulent drainage, undermining/tunneling—and escalate care per the PRIORITY‑X model.
  6. Educate patients, families, and caregivers on pressure‑ulcer prevention, off‑loading techniques, and self‑monitoring to foster adherence and reduce recurrence.
  7. Evaluate wound‑healing progress using standardized metrics (wound measurement, staging progression) and adjust the nursing plan dynamically.

  1. Prerequisites:
    • Fundamentals of skin anatomy and physiology (A&P).
    • Basic principles of fluid balance and nutrition.
    • Introductory nursing care planning.
  2. Completion Criteria:
    1. Final grade ≥ 85%, including exams, quizzes, and project.
